Self-Management - February 1, 2007 to March 31, 2008

Peer Training in Chronic Disease (Diabetes) Self-Management

 

Three people per community or organization were invited to attend a four-day training session to become peer leaders for chronic disease (diabetes) self-management. Together, these people provide two 6 week community-level sessions (2.5 hours/week) over a one year period for approximately 10 community members at a time.
