Follow these guidelines when completing the workplan.

 

Deadlines

 

The deadline to respond to the first call for submissions for the 2008/09 fiscal year is May 2, 2008. However, the NDHN will accept submissions throughout the year for consideration and depending upon available funds. Check this website regularly for updates to submission deadlines.

 

The funded workplan, including the completed evaluation section, must be returned to the NDHN no later than March 31, 2009.

 

Target Population

 

The target population for this funding is Aboriginal people in northern Ontario. Aboriginal communities or organizations targeting this population are eligible to submit a locally-relevant workplan addressing diabetes prevention or management.

 

Instructions

  1. Download the workplan form.

  1. Complete the workplan. To assist you in completing the workplan, the NDHN has provided a "sample workplan".

  1. Submit the completed workplan to the NDHN by e-mail at AboriginalDiabetes@ndhn.com or by fax at 807-737-2240.

All workplans approved for funding by the NDHN require that an evaluation component be completed. The activities outlined in the approved workplan must be complete and the evaluation must be documented in the last column of the workplan, “Actual Outcomes”. The completed workplan including the evaluation must be submitted to the NDHN before the end of the fiscal year (i.e. March 31, 2009).

 

The following activities do not meet criteria of the Northern Ontario Aboriginal Diabetes Initiative (NOADI) and do not qualify for funding by the NDHN:

  • Activities which are not Aboriginal-specific

  • Activities which do not relate to diabetes prevention, education or management

  • Activities which are not in the target region of northern Ontario

  • Capital building projects

  • Projects where research is the main focus

  • Grants to individuals

  • Ongoing or multi-year projects

Sharing Best Practices

 

A collection of “best practices” will be completed based on workplans and activities funded by the NDHN-NOADI. Your experiences can help others develop their strategies. The NDHN would be pleased to include your insights with that of other Aboriginal communities and organizations.

 

To share your best practices, simply fill out the “Sharing for Better Diabetes Prevention and Management” form (NDHN/NOADI/F005) and submit it to the NDHN by fax at 807-737-2240. The NDHN also invites you to share photos of your events provided appropriate consent has been obtained by your organization for the photos to be shared as part of the NDHN-NOADI best practice collection. Photos can be emailed to AboriginalDiabetes@ndhn.com.
