For KSDPP workshops, the host community/organization is responsible for:

1. Registration Information (the forms will be provided by the Facilitator and they must be completed by the host community or organization)

  1. Completion of registration forms

  2. Completion of evaluation forms

  3. Completion of photograph permission forms

2. Recruitment of Participants

The community needs to advertise the workshops and recruit and sign-up appropriate participants. Participants are asked to commit to attending all the days of the workshop.

 

3. Location & Meals (to be organized by the host community or organization)

  1. Meeting hall with ample space, with tables, chairs for all participants

  2. Audio-visual equipment: projector, screen, flip charts, easels and markers

  3. Water at all times

  4. Morning and afternoon snacks, beverages (menu to be planned with facilitator)

Personal Empowerment for Diabetes Prevention

People need to understand the factors that encourage and hinder us in engaging in healthy eating, physical activity and interpersonal relationships. An examination of these factors from a personal perspective can empower us to make decisions that will support healthy lifestyle change, for diabetes prevention.

 

The objectives of the two-day workshop are:

  1. Participants will understand basic information about diabetes, eating habits, physical activity habits and stress factors.

  2. Participants will examine their lifestyle patterns, activities and perceptions about health and wellness in relation to diabetes prevention.

  3. Participants will develop personal action plans to introduce changes into their lives.

  4. Participants will understand the importance of role modeling healthy lifestyles for diabetes prevention for themselves, their family and others.

The host agency must provide two lunches (menu will be planned with the facilitator).
 

Community Mobilization for Diabetes Prevention

This 5 day workshop “Community Mobilization for Diabetes Prevention” (KSDPP Training Program in Diabetes Prevention) will involve participants in:

  1. Understanding the background for successful diabetes prevention.

  2. Identifying community values and developing a community vision for diabetes prevention.

  3. Conducting an environmental scan of community strengths and weaknesses in preparation for diabetes prevention programming.

  4. Identifying goals, objectives and strategies, and a community activity calendar.

  5. Understanding teamwork and building a community coalition.

  6. Developing an information dissemination program.

Participants in this community-based training program should come from a range of community programs and services (health, education, recreation, political, economic, environmental, social, cultural, etc.). Participants will need to commit to:

  1. Five consecutive days of workshop attendance and participation.

  2. Participation in community diabetes prevention activity planning and implementation as members of a community coalition.

  3. Acting as “ambassadors of wellness” to their community and community members.

  4. A long-term process of mobilizing their community for diabetes prevention and healthy lifestyles.

Alex has stressed that the success of the Community Mobilization workshop requires full participation of education, political and recreation leaders in addition to health leadership, key workers and, of course, community members.

 

Community Mobilization for Diabetes Prevention

October 17-21, 2011 in Sudbury Ontario

The NDHN would like to invite your community or organization to apply for funding to send two people to the training. The workshop is described above. It is best if these two people include one person who is in a management and/or decision-making position (such as a Chief, Executive Director, or Health Director, for example) and one person who works either directly or indirectly with people who have diabetes or who are at risk of diabetes. A certificate will be available for participants who complete the five day training program.

 

To apply for funding to attend this workshop, complete the application form and return it to the NDHN by March 25, 2011.

School Mobilization for Diabetes Prevention

This workshop will work with school officials, teachers, parents, students and community members to develop a strategy to bring activities that teach and reinforce healthy eating habits and daily physical activity for students and teachers within a school community.

Workshop Objectives:
Upon successful completion of this workshop, participants will

Develop a vision of wellness for a school community

  1. Understand the components of an elementary school curriculum for Diabetes Prevention

  2. Outline the steps for the implementation of a diabetes prevention program in the local elementary school

  3. Conduct an environmental scan to identify community school(s) strengths and weaknesses

  4. Identify critical success factors and develop goals and objectives for school mobilization for diabetes prevention

  5. Prepare diabetes prevention activities (nutrition and physical activity) and a school calendar for students and staff

  6. Develop a plan to raise support for diabetes prevention mobilization in your school

Alex has stressed that success requires school staff participation which means that it should take place on a couple of days which have no classes scheduled for students.
 

If funding from the NDHN is approved, you and Alex McComber will be notified at which time you can make the final arrangements with Alex McComber.
