| Network of Northern Ontario Diabetes Programs
The Network of Northern Ontario Diabetes Programs (NNODP), one of two networks of diabetes programs administered by the Northern Diabetes Health Network (NDHN), consists of 39 community-based adult focused diabetes programs located across northern Ontario. | The mandate of member programs, as directed by the NDHN, is to provide quality diabetes services delivered by qualified diabetes health professionals to achieve client and community health. The primary role of the NDHN member programs is to educate people with diabetes on how to self-manage their disease. Education services offered by programs extend beyond the individual with diabetes to include family and others within the community. Program services may also include specific educational components to meet individual needs of clients such as segments that are geared towards seniors, pregnancy, intensive diabetes management, foot care, camps, support groups and community awareness events. Diabetes educators are also sensitive to the individual cultural uniqueness within First Nations, Francophone and other communities. Diabetes education may be provided through individual, group sessions or a combination of both. The multidisciplinary team, in conjunction with the client's family physician and/or specialist, consists of a Nurse Diabetes Educator and a Dietitian Diabetes Educator. Other members of the team may include a social worker, chiropodist, community support worker and healthcare providers within the communities served. |
The expertise of diabetes professionals is also made available to many surrounding locales as most of the diabetes educators travel to neighboring communities by car, and even by water and plane, to some of the more remote and isolated communities in their region. Diabetes programs work closely with members of their community to meet the needs of people with diabetes within their area. Program Advisory Committees are one example of how NDHN member programs endeavour to tailor their services to meet the needs of their clients.
