Network of Ontario Pediatric Diabetes Programs

The Network of Ontario Pediatric Diabetes Programs (NOPDP), one of two networks of diabetes programs administered by the Northern Diabetes Health Network (NDHN), consists of 34 specialized pediatric diabetes programs. The 29 secondary level programs are located in selected communities across the province to ensure reasonable access to children and youth affected by diabetes, while the 5 tertiary level programs are located in Hamilton, Kingston, London, Ottawa and Toronto. The NDHN, as per its mandate from the Ontario Ministry of Health and Long-Term Care, established and continues to administer a provincial "network" of pediatric diabetes programs. The goal of the NOPDP is to ensure that the best possible health outcomes are achieved for children and youth affected by diabetes. Each program consists of a multidisciplinary core team of Registered Nurse(s), Registered Dietitian(s) and Social Worker(s) who work in close collaboration with pediatricians, pediatric endocrinologists and other physicians, as well as other important team members, to provide comprehensive care to meet the needs of pediatric clients.

In addition to the individual and group diabetes education and support provided by the specialized multidisciplinary pediatric diabetes teams, many programs also offer a wide range of services that may include specific educational components to meet individual needs of clients, such as strategies geared towards children of specific age groups, insulin pump therapy, hosting of special events, school visits, support groups and community awareness events, only to name a few.

 


The NDHN facilitates this "network" of pediatric diabetes programs by enhancing resources, coordinating services, establishing both formal and informal linkages, providing opportunities for professional development, providing individual program support as required and monitoring/evaluating program activities.

Every secondary level program is linked to a tertiary level site to ensure that clients have access to tertiary level services when required through a consultation, shared-care and/or outreach model. The linkages between secondary and tertiary level programs are also key to the development of professional relationships and mentoring that support excellence in pediatric diabetes care across the province. Through its ongoing work on behalf children and youth with diabetes, the NDHN also facilitates and leads formal networking strategies with key stakeholders in the lives of children for the goal of working collaboratively to improve the lives of children and youth with diabetes.
