	To contribute to the Northern Diabetes Health Network Member Program in your area: 1. Click Download and Print the Donation Form 2. Fill out the form 3. Enclose your cheque 4. Mail your form and cheque to: Northern Diabetes Health Network 1204A Roland Street Thunder Bay, ON P7B 5M4 or your local Diabetes Member Program. The Northern Diabetes Health Network will issue an official tax receipt by mail for donations greater than $10. To view the PDF donation form, Acrobat Reader is required.
